Job summary

Saint Luke’s Health System is seeking a Regional Nurse Care Coordinator for its Population Health team. This RN will coordinate care for a diverse patient population, primarily by phone, to improve health outcomes and independent disease management.

The role emphasizes collaboration with multidisciplinary teams, patient education, and performance improvement activities, in a daytime, Monday–Friday schedule with an optional 50% hybrid work arrangement within the Kansas City region.

Qualifications

• 3-5 years of RN experience required or preferred
