Job summary

Community Health Center of Southeast Kansas, Inc. is seeking a Registered Nurse to provide comprehensive nursing care within the electronic health record (EHR) system and to serve as a key link between providers and patients regarding test results and medical questions.

The role includes coordinating care for upcoming appointments, reviewing immunization status and lab tests, and assisting with diabetes program needs such as refills and prior authorizations.

Qualifications

• RN license in the state where nursing practice occurs (multistate license preferred).
• Two years nursing experience in an outpatient clinic is preferred.
• BLS certification required within six months from date of hire.

Responsibilities

• Performs overall nursing care within the EHR and serves as a communication link between providers and patients on test results and questions.
• Coordinate patient care for upcoming appointments and ensure relevant health information is up to date.
• Answer triage calls and assist with scheduling and care coordination.
• Assist Diabetes Program with refills, prior authorizations, DME, and related patient needs.
• Perform other duties as assigned.
