Job summary

Comagine Health is seeking a Behavioral Health Case Manager to join our mostly remote team serving clients across the United States, with travel to the Portland, Oregon area as needed. The role demands 2+ years of clinical experience, DSM-5 diagnostic capabilities, and the ability to obtain QMHP certification within two weeks of offer.

A valid Oregon license or equivalent is required, with residency in the geographic area and frequent in-person assessments when necessary.

Qualifications

• 2+ years of clinical experience including assessments, histories, examinations, and DSM-5 diagnoses for individuals with SPMI.
• QMHP certification or ability to obtain certification within 2 weeks of offer; Oregon licensure is required or active.

Responsibilities

• Administer face-to-face assessments and develop person-centered service plans with individuals and their agents.
• Continuously monitor service plans and adjust as needed to support recovery and safety.
• Assist with transitions between community and institutional settings and coordinate related supports.
