Job summary

Moda Health seeks an RN Case Manager to perform comprehensive case management within accreditation standards for Moda members. This full-time role is remote, with pay based on experience and location. The position may be hourly depending on applicant location, and requires Oregon licensure and CCMC certification within 24 months.

Responsibilities include coordinating care plans, collaborating with care teams, and ensuring timely delivery of services. Eligible for comprehensive benefits and PTO.

Qualifications

• Graduate of accredited school of nursing; two, three or four-year program.
• Requires current unrestricted Oregon Nursing license as a registered nurse.
• Certification in case management (CCMC) required or ability to obtain within 24 months of hire.
• Must have 3 years recent hospital, home health or acute care experience.
• Proficient with Microsoft Office applications and type a minimum of 35 wpm on a computer keyboard.
• Strong analytical, problem solving, memory retention, organizational and detail orientation skills.
• Exceptional verbal and interpersonal communication skills including management of the angry customer.
• Ability to work well under pressure.
• Project a professional business image telephonically and in person.
• Ability to come in to work on time and on daily basis.

Responsibilities

• Assess, plan, implement, coordinate, monitor and evaluate cases.
• Assess: gather in-depth information to identify member needs.
• Plan: set objectives and actions to meet identified needs.
• Implement: execute case management activities to achieve goals.
• Coordination: organize, secure, integrate and modify resources.
• Monitoring: collect data to evaluate plan effectiveness and outcomes.
• Consults with physician advisors for clinical determinations.
• Serves as a resource to internal and external customers.
• Applies clinical judgment to ensure policy compliance and care standards.
• Collaborates with departments to resolve issues and drive quality improvement.
• Documents consistently, accurately and in a timely manner.
• Plans, organizes and prioritizes assignments to meet timelines.
• Performs other duties as assigned.
