Community Care Cooperative (C3) seeks a Provider Educator – Risk Adjustment & Value-Based Care to lead education programs on clinical documentation, risk adjustment coding, and population health. You will partner with health centers, providers, and leadership to improve documentation quality, coding accuracy, and performance across MSSP and MassHealth ACO programs.

You will apply CMS-HCC Version 28 knowledge and other risk adjustment models to educate and uplift provider performance, including

• Bachelor's degree in a healthcare field is required.
• CPC/CCS certification is required; CRC preferred.
• Experience with CMS-HCC risk adjustment models and MSSP/ACO programs is essential.
• Experience educating physicians and clinicians on documentation and coding best practices.
• Proficiency with EHRs, coding software, and reporting platforms is expected.

Responsabilidades

• Design, develop, and deliver provider education on risk adjustment, coding, and value-based care.
• Create training curricula, job aids, and coding reference tools for providers.
• Conduct educational reviews to identify opportunities for improvement in documentation and coding.
• Analyze coding, RAF, quality, utilization, and population health data to identify trends.
• Partner with health centers to improve documentation practices and outcomes.
• Provide coaching to underperforming providers in risk adjustment and value-based metrics.
• Collaborate with leadership to develop provider engagement and education strategies.
• Educate on CMS-HCC, DxCG, and other risk adjustment methodologies affecting Medicare/Medicaid.
