Job summary

Community Care Cooperative (C3) seeks a Provider Educator – Risk Adjustment & Value-Based Care to develop and deliver education programs focused on clinical documentation, risk adjustment, population health, and value-based care performance across MSSP ACO and MassHealth ACO contracts. You will use CMS-HCC methodologies and data insights to improve documentation quality, coding accuracy, and performance.

Qualifications

• Bachelor's degree in healthcare or related field is required.
• CPC or CCS certification required; CRC preferred.
• Experience with CMS-HCC risk adjustment methodologies is essential.
• Strong knowledge of Medicare/Medicaid value-based care programs.
• Experience in provider education and population health is required.
• Proficiency with Epic and coding/reporting tools is desirable.

Responsibilities

• Design and deliver provider education on risk adjustment and coding.
• Develop training materials, curricula, and provider resources.
• Conduct educational reviews to improve documentation and coding quality.
• Analyze coding, RAF, quality, and population health data to identify trends.
• Collaborate with health centers to improve documentation and coding outcomes.
• Provide coaching to underperforming providers in risk adjustment metrics.
• Engage leadership to implement education strategies across programs.
• Educate on CMS-HCC and related risk adjustment methodologies.
