Community Care Cooperative seeks a Provider Educator – Risk Adjustment & Value-Based Care to develop, implement, and deliver education programs focused on clinical documentation, risk adjustment coding, population health, and value-based care performance.

You will partner with health centers, providers, and leadership to improve documentation quality and coding accuracy across MSSP, MassHealth ACO, and Medicare programs.

Competenze

• Bachelor's degree in a healthcare field is required.
• CPC or CCS certification required; CRC preferred.
• Minimum 3–5 years in risk adjustment, coding, and provider education.

Mansioni

• Design, develop, and deliver provider education on risk adjustment and value-based care.
• Create materials, curricula, and job aids for providers.
• Conduct educational reviews to identify opportunities for improvement.
• Analyze coding, RAF, quality, and population health data for trends.
