Job summary

Alpine Physician Partners seeks a Clinical Risk Adjustment Coder to review records, validate HCC-related codes, and ensure CMS-compliant risk adjustment documentation. This role supports provider education and risk score accuracy across remote or hybrid settings in Colorado.

You will assign ICD-10-CM codes, perform chart reviews, and collaborate with clinicians to improve documentation quality while staying current with payer requirements and HIPAA compliance.

Qualifications

• Must hold an MD/DO/NP/PA or equivalent medical license and relevant coding certifications.
• Experience with ICD-10-CM, CMS HCC risk adjustment models, and payer guidelines is required.
• Excellent attention to detail and ability to work independently in remote or hybrid settings.

Responsibilities

• Review medical records and provider notes to identify and validate HCC and risk adjustment codes.
• Conduct in-home or virtual health assessments documenting chronic conditions for accurate coding.
• Assign and verify ICD-10-CM codes per guidelines, CMS requirements, and payer policies.
• Perform chart reviews to identify gaps in documentation and coding accuracy.
• Collaborate with clinicians to clarify documentation and support accurate chronic condition capture.
• Provide feedback to providers on documentation improvements related to risk adjustment coding.
• Monitor coding accuracy through audits and QA reviews, identifying trends and corrective actions.
• Stay updated on CMS and payer risk adjustment methodology changes affecting HCC models.
