Qualifications

• MD, DO, NP, or PA with licensure is required.
• Active coding certification: CRC, CPC, CCS, or equivalent.
• Minimum 2 years of medical coding experience with HCC/risk adjustment.
• Familiarity with ICD-10-CM and CMS HCC risk models.
• Strong medical terminology knowledge and analytical skills.
• Proficient with EHR systems and coding software.
• Ability to work remotely or in a hybrid setting.

Responsibilities

• Review medical records to identify and validate HCC diagnoses.
• Conduct home or virtual health assessments documenting chronic conditions.
• Assign and verify ICD-10-CM codes per guidelines and CMS rules.
• Perform chart reviews to close documentation gaps in risk adjustment.
• Collaborate with providers to clarify documentation for accurate capture.
• Provide feedback on documentation improvements tied to risk adjustment.
• Monitor coding accuracy via audits and quality assurance.
