Job summary

Centurion Health is seeking a Residential Counselor in Fairfield, CA for a full-time, remote-capable role within the FACT program. You will lead skills-based groups, provide psychoeducation, and connect clients to essential community resources in a structured, supportive environment.

The position requires a High School Diploma or GED, at least one year of mental health experience, and a valid CA driver’s license. CPR certification is preferred, and background checks are required.

Qualifications

• High school diploma or GED required.
• Minimum of one year of mental health experience preferred.
• Community-based mental health experience preferred.
• Must pass a background investigation; CPR certification preferred.
• Must have valid California driver’s license; insurability required.

Responsibilities

• Conduct structured skills-based groups to address coping skills (time management, goal setting, stress management, etc.).
• Provide psychoeducation through one-on-one or group sessions.
• Assist clients with connection to community services (Medical, Medicare, insurance, education).
• Address social skills, personal hygiene, communication, conflict resolution, daily living activities, job skills, transit use, and recreation.
• Support client documentation, medication management, and monitoring.
