Job summary

Vaya Health is seeking a DAC Re-entry Coordinator to support individuals with serious mental illness within 90 days of release from a Department of Adult Corrections facility. This mobile role coordinates care, housing, benefits, and health services, ensuring timely referrals and person-centered planning.

Residency must be in North Carolina or within 40 miles of the NC border with routine community travel. A Master's degree in psychology, social work, counseling, or a related field and licensure

Qualifications

  • Master's degree in Psychology, Social Work, Counseling, or similar field.
  • At least two years of progressive experience providing similar services.
  • Fully or provisionally licensed mental health clinician (LCSW/LCMHC/LPA/LMFT).

Responsibilities

  • Coordinate DAC re-entry planning and referrals to services (housing, health care, benefits).
  • Provide case management from referral through ongoing follow-up.
  • Collaborate with DAC, hospitals, DSS, and community providers to support outcomes.
  • Promote person-centered planning and discharge readiness.
  • Maintain documentation and data reporting across programs.
