Job summary

Agency for Persons with Disabilities seeks a QSI Assessor OPS to perform evidence-based assessments for eligibility and service planning using the QSI, interviewing clients, guardians, and providers, and documenting results in the online system.

The role requires a bachelor’s degree, four years of relevant experience, daily travel, and proficient computer skills; a telework option is available to support field work and coordination with regional teams.

Qualifications

• Bachelor’s degree and at least four years’ experience working with persons with developmental disabilities.
• Valid driver’s license and ability to travel daily.
• Excellent communication and computer skills to use a web-based assessment system.
• Ability to travel daily for field assessments.

Responsibilities

• Conduct evidence-based assessments using the QSI with interviews of client, guardians, and staff.
• Review central records and collateral sources to verify demographics and needs.
• Schedule meetings and follow-ups with clients and providers.
• Enter results into the QSI online application.
• Refer clients to Waiver or Pre-enrollment Coordinator for resources.
• Attend training and maintain inter-rater reliability on the QSI.
