Job summary

PossAbility, LLC is seeking a Qualified Intellectual Disability Professional (QIDP) to oversee and manage Individual Service Plans (ISPs) for individuals with diverse disabilities and abilities. You will develop behavior support plans, monitor progress, and collaborate with the care team to ensure rights and needs are met.

You will conduct monthly program reviews, coordinate data collection, and participate in interdisciplinary collaboration to ensure regulatory compliance with IDHS and related

Qualifications

• A bachelor's degree in healthcare, public health, or related field.
• QIDP certified and on the Department’s QIDP database
• Five or more years of experience as a direct care worker for individuals with IDD
• Knowledge of home care regulations and best practices
• Strong leadership and communication skills
• Excellent organizational and problem-solving abilities
• Valid driver's license and proof of insurance
• Current First Aid and CPR certifications

Responsibilities

• ISP development and implementation to meet individuals' needs and rights.
• Develop and oversee BSPs to address challenging behaviors.
• Prepare monthly written program reviews with data and updates.
• Collaborate with therapists, nurses, and social workers.
• Ensure regulatory compliance with IDHS and related agencies.
• Serve as the primary case manager for supported individuals.
• Provide staff training and supervision as needed.
