Cleveland Clinic Foundation is seeking BE/BC Family Medicine, Internal Medicine, or Med/Peds physicians to join our fully remote Primary Care Connect team. This virtual-first program provides timely access for new primary care patients and ongoing virtual care across the system.

You will conduct video visits, address acute and chronic needs, and help patients transition to the best long-term care model across the Cleveland Clinic system.

Qualifications

• Ambulatory care experience in primary care settings.
• Comfort with digital tools and EMR systems.
• Passion for patient-centered care and innovation.

Responsibilities

• Conduct video visits with new and existing patients.
• Address acute and chronic needs and close preventive care gaps.
• Support transitions to appropriate long-term care models.
• Collaborate with APPs and interdisciplinary teams.
