Job summary

Novant Health is seeking a board-certified Family or Internal Medicine physician for a fully remote, employed Primary Care Physician role. You will manage your own patient panel, delivering video-based preventive, acute, and chronic care in a fully remote setting, with 40 hours per week and 36 clinical hours.

Work from home with support from K Health's AI platform and Care Concierge team; NC/SC license required; Mondays and Fridays must be worked; experience with EPIC EMR preferred;

Qualifications

• Board-Certified in Family or Internal Medicine.
• 3+ years of post-residency work experience.
• 2+ years in-person primary care setting.
• Active NC or SC medical license (IMLC preferred).
• Experience with EPIC EMR.

Responsibilities

• Provide high-quality preventive, acute, and chronic care in a fully-remote, virtual setting.
• Manage a dedicated panel of patients as their primary care physician.
• Refer patients to specialists or acute care services when clinically indicated.
• Work on population health initiatives for risk-attributed patients (AWVs, care gaps, transitions of care, risk capture).
• Support patients immediately post-discharge to improve understanding of treatment plans and reduce hospital re-admissions.
• Practice continuous self-evaluation to ensure adherence to clinical guidelines and best practices.
