Belle is redefining primary care for Medicare members through in-home and virtual care. We seek a Board Certified/Eligible Primary Care Physician to deliver preventive, telehealth-based care for seniors, collaborating with an integrated team to create personalized care plans and manage chronic conditions.

Qualifications

• MD or DO, Board Certified/Eligible in Family Medicine, Internal Medicine, or Geriatrics.
• Active medical license; multi-state licensure a plus.
• Credentialed with major plans: UHC, Aetna, Humana.
• Tech-savvy with telehealth experience and excellent communication skills.

Responsibilities

• Provide routine telehealth visits with Medicare patients (E/M and Chronic Care Management).
• Create and manage personalized care plans for ongoing conditions.
• Collaborate with our integrated care team (nurse case managers, NPs, CHWs).
• Ensure thorough documentation in our EHR and compliance with quality measures.
• Engage patients in preventive care to reduce avoidable complications.
