Job summary

Belle is redefining primary care for Medicare members via in-home preventive foot care and telehealth. We are seeking a Board Certified (or Eligible) Primary Care Physician to deliver high-quality, preventive-focused care through telehealth for seniors.

The role is 100% remote, Monday–Friday, with no on-call requirements, and offers comprehensive benefits and strong career growth within a mission-driven team.

Qualifications

• MD or DO, Board Certified/Eligible in Family Medicine, Internal Medicine, or Geriatrics
• Active medical license — multi-state licensure a plus
• Credentialed with major plans: UHC, Aetna, Humana
• Strong interest in preventive care, senior health, or chronic condition management
• Tech-savvy and comfortable practicing via telehealth
• Collaborative, mission-driven mindset with excellent communication skills

Responsibilities

• Provide routine telehealth visits with Medicare patients (E/M and Chronic Care Management)
• Create and manage personalized care plans for ongoing conditions
• Collaborate with our integrated care team (nurse case managers, NPs, CHWs)
• Ensure thorough documentation in our EHR and compliance with quality measures
• Engage patients in preventive care to reduce avoidable complications
