Belle, LLC is seeking a Board Certified (or Eligible) Primary Care Physician to deliver preventive-focused care through telehealth for Medicare seniors. This 100% remote role offers in-home and virtual care coordination via AI-enabled telehealth, with flexible hours and a strong benefits package including health, dental, vision, 401k, PTO and paid holidays.

Formación

• MD or DO, Board Certified/Eligible in Family Medicine, Internal Medicine, or Geriatrics.
• Active medical license — multi-state licensure a plus.
• Credentialed with major plans: UHC, Aetna, Humana.
• Strong interest in preventive care, senior health, or chronic condition management.
• Tech-savvy and comfortable practicing via telehealth.
• Collaborative, mission-driven mindset with excellent communication skills.

Responsabilidades

• Provide routine telehealth visits with Medicare patients (E/M and Chronic Care Management).
• Create and manage personalized care plans for ongoing conditions.
• Collaborate with our integrated care team (nurse case managers, NPs, CHWs).
• Ensure thorough documentation in our EHR and compliance with quality measures.
• Engage patients in preventive care to reduce avoidable complications.
