Included Health is seeking board-certified Family Nurse Practitioners to join our Virtual Primary Care team. You will deliver comprehensive care to patients of all ages via video and phone visits, guiding them on how and why their care plans support health outcomes.

The ideal candidate manages their own patient panel, collaborates with a multidisciplinary team, and communicates in patient-friendly language.

Qualifications

• Active, unrestricted NP license in your state.
• Board certification by ANCC or AANP.
• Minimum four years of ambulatory primary care experience.
• Experience managing a patient panel as the primary care provider.

Responsibilities

• Provide high-quality virtual primary care through video visits, including assessment, diagnosis, treatment, and patient education.
• Manage a panel of patients, delivering comprehensive care across acute, chronic, and preventive needs.
• Collaborate with Care and Case Management teams to develop care plans and support outcomes.
• Partner with supervising physicians as required by state and practice guidelines.
• Work with Credentialing Team to obtain additional state licenses as needed.
• Maintain patient satisfaction and quality metrics by practicing evidence-based care.
