Qualifications

Responsibilities

• Coordinate intake of cases to expedite Care Coordination and triage to resources.
• Initiate and monitor Care Coordination plans and status updates.
• Research and maintain library of resources, guidelines, and policies.
• Support onboarding for new employer groups with onsite staff.
• Coordinate care gap outreach and verify list accuracy per protocol.
• Train staff on information systems, referrals, and policy procedures.
• Develop and generate reports; summarize data for leadership and partners.
• Assist in stocking materials and supporting Population Health activities.
