Job summary

Elite Home Health, part of LHC Group and aligned with Optum, is seeking a PRN Physical Therapist Assistant to perform treatment and provide exercise instruction and patient education under a supervising Physical Therapist’s plan of care. This role emphasizes patient well‑being and safe home‑care delivery.

You’ll work remotely from anywhere in the U.S. with flexibility, collaborating with a multidisciplinary team while adhering to state laws and telecommuter policies.

Qualifications

• Current, licensed, registered, or certified to assist in the practice of physical therapy in the state of practice.
• Current driver’s license and vehicle insurance; access to a dependable vehicle, or public transportation.
• Available to work a weekend rotation.
• CPR certification or ability to complete within 90 days of hire.
• Ability to perform physical tasks required for patient care, including lifting, transferring, and assisting with mobility.

Responsibilities

• Follow state-specific laws governing the provision of physical therapy in home care and follow the supervising PT plan of care.
• Instruct and aid patients in active and passive exercise, muscle re-education, gait, functional activities, and transfer.
• Observe and report patient responses to treatment to the supervising PT and care team.
• Educate patient, family, and caregivers on therapy within the PTA scope.
• Function in diverse home settings and uphold professional standards.
