Job summary

Intermountain Health is seeking a Pharmacy Technician for the Trinsic Clinical Integrated Network to focus on improving medication adherence and patient outcomes. Qualifications

• Completion of pharmacy technician training program or at least two years of experience as a pharmacy technician.
• Current pharmacy technician certification and/or license in state of practice.

Responsibilities

• Collaborates with pharmacists and other healthcare professionals to manage medications, improve adherence, and address medication-related issues that impact the overall health of a population.
• Supports value-based care programs by monitoring medication adherence, identifying gaps in care, and supporting interventions aimed at improving health outcomes and reducing costs.
• Assists in medication reconciliation, ensuring patients have accurate medication lists, especially during transitions of care.
• Educates patients and their families about medications, potential side effects, and adherence through calls or other communication methods.
• Generates reports on medication adherence, therapeutic outcomes, and other relevant metrics to track progress.
