Job summary

PharmD Live in the United States is seeking a Pharmacist to support patients across Western and Central states through remote clinical services. The role focuses on medication therapy management, patient education, and coordination with healthcare teams to drive better clinical outcomes.

Qualifications

• Doctor of Pharmacy (PharmD) degree.
• Active pharmacist license in at least one listed state.
• Willingness to obtain additional licensure as required.

Responsibilities

• Perform Medication Therapy Management (MTM) interventions
• Complete medication reconciliation and identify optimization opportunities
• Provide patient-centered education on medications and disease management
• Support chronic care initiatives (e.g., diabetes, hypertension, COPD, CHF)
• Collaborate with providers on treatment planning and medication adjustments
• Detect and resolve medication-related risks and discrepancies
• Ensure timely and accurate documentation of encounters
• Facilitate smooth care transitions and ongoing care coordination
