Job summary

Lumicera is seeking a Pharmacist, Clinical Consultation to join our team. The main duty is to perform telephonic consultations with patients regarding the use of their specialty medication and management of their disease state.

The Pharmacist will perform medication reconciliation, screen for drug interactions, provide side effect management, contact prescribers for clarification, and expedite order processing while meeting department KPIs and service levels.

Qualifications

• Graduate of an ACPE Accredited School of Pharmacy with a B.S. Pharmacy or PharmD degree.

Responsibilities

• Provide telephonic patient education on specialty medications and disease state management.
• Document and report patient consultations and interventions in pharmacy software per policy.
