Lumicera Health Services, powered by Navitus, is hiring a Pharmacist, Patient Consultation Services, to provide telephonic clinical consultations for high-cost specialty medications. This part-time role offers two 10-hour shifts per week or five 4-hour shifts, with remote work available to US residents.

Competenze

• Graduate of an ACPE-accredited School of Pharmacy with PharmD or BS in Pharmacy.
• Current, unrestricted Pharmacist license in the state of residence or eligible for reciprocity within 4 months.
• Experience counseling patients and caregivers on medication therapy and disease state management.
• Experience documenting clinical assessments and interventions in pharmacy systems.
• Ability to identify drug interactions and adherence barriers.

Mansioni

• Conducts telephonic clinical consultations to support safe use of specialty medications.
• Performs patient assessments and medication reconciliation to identify adherence barriers and drug interactions.
• Provides education on disease state management, medication administration, storage, treatment expectations, and side effects.
• Answers clinical questions from patients and caregivers to aid informed treatment decisions.
• Identifies patient-specific clinical concerns and escalates findings to promote continuity of care.
• Documents patient consultations, assessments, interventions, and recommendations in pharmacy systems.
• Maintains productivity, quality, and service level standards through timely completion of consultations.
• Supports quality improvement through reporting of medication errors and safety concerns.
