Lumicera is seeking a Pharmacist, Clinical Consultation to join our team. The role focuses on telephonic consultations with patients using specialty medications and managing disease states.

The Pharmacist will perform medication reconciliation, screen for drug interactions, provide side-effect management, and coordinate with prescribers to expedite orders. Remote work is available for many locations. This position offers a 7.5% annual bonus and comprehensive benefits.

Formación

• ACPE-accredited PharmD or B.S. Pharmacy degree.
• Current, unrestricted pharmacist license in home state with ongoing education.
• At least two years of practice as a licensed Pharmacist preferred.
• Strong clinical background; experience with specialty medications desirable.
• Commitment to compliance and patient safety.

Responsabilidades

• Provide telephonic patient consultations for specialty medications and disease state management.
• Document consultations and interventions in pharmacy software per policy.
• Interpret prescriptions, verify drug strength/directions, and verify CDS validity.
• Coordinate with physicians for new prescriptions and transfers; promote generics when possible.
• Verify system data entry; respond to clinical questions from the pharmacy team.
• Stay current with specialty therapies and participate in process improvements.
• Back up pharmacists and train junior staff as needed.
• Report medication errors per policy and adhere to regulatory standards.
• Uphold laws and ethical standards; perform other duties as assigned.

Conocimientos

ACPE Accredited Pharmacy degree

Active pharmacist license

2+ years experience
