Job summary

Your Health Organization seeks a Pharmacist to join our growing primary care team serving the Marietta area. You will dispense medications, counsel patients, and support care plans across telehealth and in-person visits, with a focus on safe therapy and adherence.

You will collaborate with interdisciplinary teams to optimize medication therapy, manage MTM services, and coordinate transitions of care, while upholding high standards of patient-centered care and safety.

Qualifications

• PharmD from an accredited program; active pharmacist license.
• Clinical pharmacy experience preferred.
• Strong clinical knowledge and evidence-based practice.
• Ability to coordinate care and manage multiple patients.

Responsibilities

• Ensure accurate dispensing and distribution of medications per regulations.
• Counsel patients on medication use and potential side effects.
• Monitor therapy outcomes and adjust therapy as needed.
• Provide MTM services, including medication reviews and reconciliation.
• Conduct telehealth or in-person patient consultations.
• Collaborate with care teams to develop comprehensive care plans.
