Job summary

PharMerica, an affiliate of BrightSpring Health Services, seeks a Pharmacist Consultant to support long-term care facilities remotely across Pittsburgh, Erie, Hermitage, and Meadville. You will evaluate orders, optimize therapy, lead care teams with prescribers and nursing staff, and help reduce drug spend while ensuring regulatory compliance.

Qualified candidates hold a PharmD or Bachelors in Pharmacy with an active license; immunization certification is required.

Qualifications

• Bachelor's degree from an accredited School of Pharmacy or PharmD.
• Current license required. Dual license preferred.
• Immunization Certification required (e.g., through APhA).
• One-year experience in acute, long-term care or home health preferred.
• Consultant or Clinical Pharmacist experience preferred.
• IV clinical experience preferred.
• Must be able to work weekends or outside normal business hours when needed.
• Ability to lift patient charts repetitively and stand for long periods.
• Good customer service skills and knowledge of state/federal long-term care regulations.

Responsibilities

• Evaluates physician medication orders for drug appropriateness, dosage, interactions, route, and stability; makes therapy recommendations to physicians and facility staff.
• Reviews drug regimens and clinical indicators for residents monthly and reports irregularities with recommendations.
• Documents all communication with physicians, nurses, and residents/families.
• Maintains knowledge of pharmacy laws and long-term care regulations.
• Works cooperatively as a team member.
• Maintains knowledge of pharmaceutical care practices for long-term care residents.
