Job summary

PharMerica is seeking a Pharmacist Consultant to support long-term care and senior living clients from a remote location within designated US territories. The role focuses on clinical services, medication optimization and regulatory compliance to help facilities improve care and reduce costs.

Qualifications

• Pharmacy degree (PharmD) and current license required.
• 1 year or more experience in acute, long-term care or home health preferred.
• IV clinical experience preferred.
• Consultant or Clinical Pharmacist experience preferred.
• Ability to work weekends or outside normal hours when needed.
• Ability to lift patient charts repetitively.
• Stand for long periods of time.
• Good customer service skills.
• Understanding of state and federal regulations for long-term care.
• Understanding of formulary management.

Responsibilities

• Evaluates physician medication orders for drug appropriateness, dosage, interactions and administration.
• Reviews resident drug regimens monthly and reports findings to nursing and physicians.
• Documents all communication with physicians, nurses, and patients/families.
• Maintains knowledge of pharmacy laws and LTC regulations.
• Works cooperatively as a team member.
• Maintains knowledge of pharmaceutical care practices for LTC residents.

Education

Bachelor's degree from an accredited School of Pharmacy or Pharmacy Doctorate
