Holon Health is seeking a Recovery Coach to support justice-involved individuals on their recovery journeys. As a certified PRS, you will offer encouragement, build trust, and help navigate barriers to wellness, using your lived experience as a powerful asset.

This full-time, remote position requires compassionate guidance and collaboration with the Holon care team to develop personalized recovery plans and connect clients to vital community resources.

Responsibilities

• Build trusting relationships with clients using lived experience
• Provide hope, motivation, and accountability from recovery perspective
• Collaborate with Holon care team to create personalized recovery plans
• Connect clients to housing, healthcare, employment, transportation resources
• Conduct strengths-based assessments to identify needs and goals
• Advocate for clients within justice and care systems
• Partner with Care Navigators and clinicians for integrated care
• Stay informed on best practices in peer support and recovery-based care
• Maintain confidentiality and HIPAA compliance
• Contribute to new initiatives and engagement strategies
