Responsibilities

• Schedule and coordinate patient appointments across the care team using scheduling systems.
• Gather, organize, and maintain patient intake forms and histories in the EHR.
• Communicate with referring providers and patients about outcomes and updates.
• Reduce no-show rates via reminders and proactive outreach.
