Imagine Pediatrics is a tech-enabled pediatric group providing 24/7 virtual and in-home medical and behavioral care. This remote Behavioral Health Care Manager role requires a Masters in social work and an active LMSW with licensure in FL, NJ, or NC.

You will manage care for medically complex children, coordinate with clinicians and families, and support crisis interventions. The role offers a base salary range of $70,000–$77,000 plus annual bonus, competitive benefits, and travel for training.

Qualifications

• Masters degree in social work or related field.
• LMSW license required.
• Active FL, NJ or NC license required.
• Minimum 3–5 years post-graduate health care social work/case management experience.
• Experience with telehealth services.

Responsibilities

• Provide case management for medically complex children enrolled in the behavioral health program.
• Monitor high-risk pediatric patients up to 21 years old and adjust care plans as needed.
• Conduct biopsychosocial assessments addressing behavioral, social, and family needs.
• Coordinate and evaluate patient/family care plans with clinicians and schools.
• Deliver crisis interventions and connect families with social services as appropriate.
• Collaborate with external care teams regularly to support patient progress.
