Job summary

Vaya Partners is seeking a Registered Nurse to perform needs-based assessments for State Plan Personal Care Services in beneficiaries' homes and community settings across NC counties. The PCS Assessor will schedule, conduct telephonic and face-to-face assessments, coordinate with prescribers, and submit SARs on behalf of PCS providers, while maintaining confidentiality and quality service.

Residency must be in North Carolina or within 40 miles of the NC border, with travel to multiple counties

Qualifications

• Bachelor's degree in Nursing or related healthcare field and NC RN licensure.
• Two years of home care or PCS assessment experience preferred.
• Experience with PCS assessments and Medicaid policies is preferred.

Responsibilities

• Schedule assessments with members and families.
• Conduct needs-based PCS assessments in homes or community settings.
• Submit service authorization requests (SARs) on behalf of PCS providers.
• Maintain privacy and dignity of beneficiaries and comply with HIPAA.
• Collaborate with prescribers and healthcare professionals as needed.
• Travel to counties as required.
