Overview

The Case Development Coordinator: is responsible for evaluating, reviewing, and following all vented referrals, including phone and electronic referrals. The Specialist provides customer service to referring hospital partners and promotes communication to increase organ and tissue donation opportunities and outcomes.

Supports and fosters the mission, vision, and core values of LiveOnNY and its commitment to organ, eye, and tissue donation.

Remote position. Must reside in NY.

Shifts: 3pm-12am

Responsibilities

• Participate in scheduled department meetings and training.
• Serve as the principal source of donor information throughout the referral case development process.
• Screen and triage vented referrals for medical suitability via record review.
• Complete donor evaluation using electronic donor records and communicate with healthcare providers and families.
• Provide timely feedback about referrals and hospital plans to supervisors and staff.
• Document and ensure quality assurance per policy in a timely manner.
• Maintain skills through training, evaluations, and continuing education.
• Identify barriers to donor potential, develop strategies to minimize risks, and escalate high-risk situations.
• Communicate with healthcare teams to obtain patient information and family understanding.
• Follow up with hospital partners on case plans and outcomes.
• Identify when clinical and authorization triggers are met and communicate next steps.
• Contribute to case reviews and process improvements.
• Participate in continuous improvement processes.
• Perform other duties as assigned.

Qualifications

• Associate or Bachelor's degree in nursing.
• Minimum 1 year in Med-Surg required.
• CPTC certification preferred.
• Understanding of organ and tissue donation process; intermediate computer skills required.
• Strong organizational skills and ability to multitask.
• Ability to analyze situations, create solutions, and collaborate effectively.
