Job summary

Upstream Rehabilitation is seeking a Remote Patient Intake Specialist to coordinate referrals, gather patient information, and schedule appointments for therapy or testing across our nationwide network.

You will work with case managers and clinics to ensure a seamless patient experience, while maintaining organized scheduling and accurate documentation. This role emphasizes strong communication and the ability to multitask in a fast-paced environment.

Qualifications

• High school diploma or equivalent.
• Experience helping customers or patients in a phone-based environment.
• Strong communication and relationship-building skills.
• Ability to stay organized, multitask, and solve problems calmly and professionally.
• Comfortable using Microsoft Office and learning new systems and tools.

Responsibilities

• Schedule evaluations, treatments, and specialized testing (e.g., FCEs) promptly and accurately.
• Review referrals and medical documentation to ensure successful appointments.
• Match patients to the right clinic or specialist using internal tools.
• Serve as a key contact for case managers, attorneys, and partners with clear, timely communication.
• Monitor scheduling activity, identify trends, and work with the team to resolve issues.
• Build strong relationships with clinics and referral partners for a seamless patient experience.
• Stay organized, track outreach, and help the team meet goals.
• Undertake other projects and duties as assigned.
