Qualifications

Responsibilities

• Conduct discharge and aftercare calls with guardians for all sites.
• Collect, organize, and analyze patient care data and present findings via video or in person.
• Manage data input and evaluation within Qualtrics.
• Collaborate with departments to develop enhanced monitoring and data collection.
• Participate in Performance Improvement initiatives to evaluate best practices.
• Maintain a professional, secure home office environment and protect information.
