Thyme Care is seeking a Palliative Care Nurse Practitioner to join its clinical team. You will see members for specialty palliative care outpatient visits via video, participate in interdisciplinary care planning, and educate the team on palliative care practices.

The role offers remote or hybrid work in New York City or Nashville. Ideal candidates have 3+ years as an NP with at least 2 years in palliative care, ACHPN certification, and a willingness to obtain additional state licenses.

Qualificações

• 3+ years NP experience with at least 2 years in palliative care.
• ACHPN certification required.
• Compact RN license and willingness to obtain additional state licenses.
• Strong communication and organizational skills.
• Experience in outpatient oncology setting is a plus.

Responsabilidades

• Provide outpatient palliative care via video visits as part of an interdisciplinary team.
• Oversee and direct interdisciplinary case conferences for members with advanced serious illness.
• Educate the team on palliative care and advance care planning.
• Report to Senior Medical Director; role can be remote or hybrid in NYC or Nashville.
