Job summary

Qualifications

• Active, unrestricted compact RN license is required.
• New York, Illinois and/or Michigan licensure preferred.

Responsibilities

• Conduct comprehensive symptom assessments for members with serious illness.
• Provide longitudinal support to members and their care networks.
• Facilitate end-of-life care discussions when appropriate.
• Address clinical issues and escalate as needed.
• Participate in case conferences to monitor member progress and coordinate care.
