Job summary

HarmonyCares is seeking a Nurse Practitioner to deliver compassionate, home-based primary and palliative care to medically complex patients in Queens, NY. Qualifications

• Active Nurse Practitioner license in NY.
• Experience in primary and palliative home-based care preferred.
• Strong assessment and care-planning skills.
