Job summary

Tuesday Health is seeking a Palliative Care Advanced Practice Provider to lead clinical delivery in partnership with the leadership team. This remote role supports patients and families, coordinating care across our network to ensure high-quality, value-based palliative services.

The ideal candidate holds an MSN or PA degree, Texas licensure, APRN certification, and DEA license, with 5+ years in palliative or hospice care and a proven ability to collaborate with multi-disciplinary teams.

Qualifications

• MSN or PA degree and active Texas license.
• APRN board certification and DEA license.
• Minimum 5 years in palliative care or hospice.

Responsibilities

• Lead internal and joint case rounds to ensure care plans are followed.
• Coordinate with network providers to optimize services.
• Review population data and implement clinical interventions.
• Support training for providers and staff.
• Contribute to quality improvement initiatives.
• Oversee performance of network partners and provide guidance.
