Job summary

Cardinal Health is seeking a Nurse, Individualized Care Nurse Care Management to coordinate patient-centered services remotely. You will bridge patients, providers and support systems to promote continuity of care and adherence.

The role requires RN licensure in California, 5 years of clinical experience, and ability to manage a large caseload while coordinating with multidisciplinary teams. Remote work option is available with standard schedule.

Qualifications

• Registered Nurse with current CA license.
• 5 years’ hospital or clinical experience in chronic disease care preferred.
• Experience in care management or oncology a plus.

Responsibilities

• Coordinate medical treatments and patient education across care teams.
• Manage caseload of 100–150 members.
• Document encounters accurately in the EHR and ensure HIPAA compliance.
• Collaborate with onsite and remote care teams to support transition of care and follow-up.
