Job summary

Cardinal Health is seeking an Individualized Care Nurse Care Manager to coordinate medical treatments, educate patients, and facilitate communication among patients, providers, and support systems in a remote, nationwide setting.

You will bridge gaps in care, promote continuity, and support patients toward optimal health outcomes, with emphasis on chronic disease and oncology care, HIPAA compliance, and team collaboration.

Qualifications

• Registered Nurse with a current, unrestricted Wyoming license or compact license including Wyoming.
• 5 years’ hospital or clinical experience with complex chronic disease states preferred; oncology experience favored.
• Care Management experience is strongly preferred; heavy oncology patient experience favored.
• Experience with Microsoft Office products.

Responsibilities

• Collaborate with health care staff to develop, implement, monitor and evaluate clinical care and coordinate care management.
• Manage a caseload sized between 100-150 members and coordinate care transitions.
• Document encounters and activities in the designated system in a timely manner and ensure HIPAA compliance.
• Coordinate with oncology care teams to assess patient needs and modify care as needed.
• Lead quality improvement activities by identifying trends and barriers to program outcomes.
• Attend meetings, seminars and conferences and communicate provider instructions and patient education materials.
