Cardinal Health seeks a Nurse Care Manager to provide structured, patient-centered care coordination, education, and bridging between patients and providers. You will manage complex cases, collaborate with oncology teams, and ensure high-quality care delivery across care transitions.

The role emphasizes care management for chronic disease, especially oncology, with remote work options and a strong focus on HIPAA compliance and documentation.

Qualifications

• Registered Nurse with a current, unrestricted California license.
• 5 years’ hospital or clinical experience with complex chronic disease states preferred.
• Care Management and oncology experience strongly preferred.
• Proficient with Microsoft Office; excellent verbal and written communication.
• Able to work collaboratively in a team; strong organizational skills; bilingual a plus.

Responsibilities

• Collaborate with health care staff to develop and coordinate care plans.
• Maintain knowledge of chronic disease management including cancer.
• Ensure compliance and accurate documentation with federal/state regulations.
• Document encounters in the designated system accurately and timely.
• Participate in case conferences and team huddles for coordinated care.
• Identify patients for case management and assess needs with the oncology team.
• Develop and review clinical pathway trends with service teams.
• Support quality improvement by identifying trends and barriers.
• Maintain HIPAA compliance.
