Qualifications

• 5 years of clinical or case management experience.
• Experience with diverse populations.
• Training in CBT/MI/DBT.
• RN licensure or multi-state licensure (eNLC) preferred.
• Ability to coordinate with physicians and care teams.

Responsabilités

• Manage a caseload of members with complex health needs.
• Develop and implement individualized care plans.
• Coordinate services and monitor progress toward goals.
• Advocate for cost-effective, quality outcomes.
