Job summary

Highmark Inc. is seeking a Case Manager to deliver comprehensive case management services for members, coordinating care plans and monitoring progress to ensure quality and cost-effective outcomes.

Responsibilities include assessments, care planning, collaboration with physicians and providers, and advocacy to meet members’ health care needs within contract benefits. Remote/office-based option may be available.

Qualifications

• Five years in clinical, case/utilization management, or health insurance roles.
• RN licensure in state or multi-state via eNLC.
• CCM certification within 36 months of hire (exempt for current by Aug 2026).

Responsibilities

• Manage a caseload of members with complex health needs.
• Conduct comprehensive assessments and develop care plans.
• Assess health status, needs and resources.
• Develop and coordinate individualized care plans with providers.
• Implement and monitor care plans and adjust as needed.
• Collaborate with physicians and the health care team.
• Document activities per policy and regulatory standards.
