Qualifications

• Must hold current RN licensure in state of practice or multi-state eNLC licensure.
• CCM certification must be obtained within 36 months of hire.
• Experience with diverse populations and complex health needs is required.
• Advanced training in CBT, motivational interviewing, or DBT is preferred.
• Proficiency with EMR systems and case management software expected.

Responsibilities

• Manage a caseload of members with complex health needs.
• Conduct comprehensive assessments and develop complex care plans.
• Assess health status, needs and resources of members.
• Coordinate and implement care plans with providers and members.
• Monitor progress and adjust plans to achieve goals.
• Advocate for members and coordinate access to services.
• Document activities in compliance with policies and regulations.
• Provide mentorship to junior Case Managers.
