Job summary

Requirements include RN licensure (or multi-state via eNLC), CCM certification within 36 months, and advanced CBT/MI/DBT training or experience.

Qualifications

• 5 years in clinical, case/utilization, or disease management work.
• Experience with oncology, transplant, maternity, NICU, or pediatric care.
• Experience serving diverse populations.
• Advanced training in CBT, motivational interviewing, or DBT.
• RN licensure required.
• Strong clinical assessment and care planning skills.
• Proficient with MS Office and case management software; solid EHR skills.

Responsibilities

• Manage a caseload of members with complex health needs and advanced clinical requirements.
• Conduct comprehensive assessments and develop complex care plans.
• Assess health status, needs, and available resources for members.
• Collaborate with members, physicians, and other providers to create care plans.
• Coordinate and implement care plans; ensure access to services and resources.
• Monitor progress and adjust plans as needed to meet goals.
• Advise and mentor lower-level Case Managers; serve as clinical resource.
• Document all activities per policies and regulatory requirements.
• Communicate clearly with physicians, providers, and team members.
