Qualifications

• Advanced training in CBT, DBT or related therapies.
• Proficiency with Microsoft Office and case management software.

Responsabilités

• Manage a caseload of members with complex health needs and coordinate care.
• Conduct comprehensive assessments and develop care plans.
• Monitor progress and adjust plans to meet goals.
• Collaborate with physicians and healthcare providers to implement plans.
• Document activities in compliance with policies and regulations.
• Mentor lower-level Case Managers and provide clinical guidance.
