Job summary

Blue Ridge Cancer Care is seeking an experienced Collaborative Practice Nurse (CPN) to support oncology patients across the continuum of care. The CPN will act as a clinical resource and care coordinator, partnering with providers, nursing teams, and interdisciplinary staff to improve outcomes and patient experience.

This role emphasizes proactive patient management, education, and navigation of complex needs.

Qualifications

• Bachelor's prepared Registered Nurse (BSN) preferred; Oncology Certification (OCN) a plus; Nurse Navigator Certification a plus.
• Five years of RN experience; oncology experience preferred.
• Salary scale indicates pay range for BRCC within the stated region.

Responsibilities

• Respond promptly to patient inquiries via calls, portal messages, and emails with timely intervention.
• Assess and manage patient symptoms using protocols and document interventions.
• Collaborate with physicians via electronic communication for urgent cases.
• Coordinate outpatient visits to ensure continuity of care.
• Lead Patient Care Conferences and communicate outcomes to the care team.
• Perform risk stratification and psychosocial assessments; provide support and referrals as needed.
• Educate about oral chemotherapy, monitor side effects, and support adherence.
• Contribute to survivorship planning and advance care discussions.
• Support quality initiatives and navigation metrics reporting; mentor junior staff.
