Job summary

Cardinal Health is seeking a qualified Nurse Care Manager to deliver patient-centered care remotely across the United States. The role coordinates medical treatments, delivers patient education, and ensures smooth communication among patients, oncology teams, and support networks to promote optimal health outcomes.

Requirements include an active RN license (Wyoming or compact), 5+ years of hospital/clinical experience with chronic disease, and strong documentation and communication skills.

Qualifications

• Registered Nurse with a current, unrestricted Wyoming license or compact license including Wyoming.
• 5 years’ hospital or clinical experience with complex chronic disease states preferred.
• Care Management experience is strongly preferred.
• Heavy oncology patient experience is strongly preferred.
• Experience with Microsoft Office products.
• Superior communication skills to include verbal and written.
• Must be able to work collaboratively; team focused.
• Excellent organizational skills.
• Bilingual skills a plus.

Responsibilities

• Collaborate with health care staff to develop, implement, monitor and evaluate clinical care and coordinate care management activities.
• Coordinate care management for chronic disease patients including oncology care.
• Document all encounters and activities accurately in the designated system.
• Participate in interdisciplinary case conferences and team huddles to ensure coordinated care.
• Assist in quality improvement activities by identifying trends and opportunities to improve program outcomes.
• Communicate oncology provider instructions and patient education materials.
